This is a useful technique that can help you give your repentances more meaning. It's very simple.
All you have to do is get a notebook, or piece of paper, or a new note on a device. Name it "repentance tracker."
Then, when you repent, you have the option of writing down what you repented from on that day, and potentially possible solutions.
For example:
You can just write the date and the sin you want to avoid.
As time goes on and you can record more and more repentances and sins you want to avoid, it can be a great help and a way to stay organized in your dedication to Allah.

Conditions of repentance (tawba): According to Islam Q & A, the conditions of repentance are:
"1- You should give up the sin
2- You should regret having done it
3- You should resolve never to go back to it.
"If one of these three is missing, then your repentance is not sincere. If the sin has to do with the rights of another person, then there are four conditions: the three mentioned above and restoring the rights of that person."

(not all details of repentance in various situations are mentioned here; search further if you are unsure about something - also I would not recommend Islam Q & A generally - I find that Dar al-Ifta al Misriyya is more balanced and logical while Islam Q & A is extreme)
